Individualized Health Services at Charter House
Founded on evidence-based best practices, our Individualized Health Services are a proactive wellness program, helping you maintain your independence and enhance your quality of life while living at Charter House. Available only to current residents, this personalized service allows you to remain comfortably in your Independent Living apartment while receiving the care you need to continue living a full and vibrant life. From nurse visits to medication management support to medical appointment escorts, you and your loved ones can take comfort in our program’s convenient, supportive medical and health services.
Personal Aide Services
Personal Aide services allow residents to get the extra help they need from the comfort of their own apartment. Our friendly Personal Aides are available to provide everything from bathing, dressing and grooming assistance to help with homemaking tasks, simple meal preparation, and medication reminders. Your Personal Aide is there to help make your days stress-free.
Nursing Care Services
Our Individualized Health Services program’s nurses are available to residents to provide care coordination, medication management, and fall assistance. You and your loved ones will have peace of mind that your in-home care is provided by Mayo Clinic’s highly skilled and licensed nurses.
Next-Level Care
Catered Living Center
After consultation with loved ones and healthcare team, current residents may choose to progress along the continuum of care to our Catered Living Center. Catered Living Center offers access to our personal care services as well as our high-quality medical services, with a focus on helping residents safely maintain the highest level of independence possible.
Our Catered Living Center provides an inclusive package designed to meet your needs.
The Catered Living Center package at Charter House includes 24/7 personal care services delivered by our on-site health services staff. Our skilled nursing staff will manage your clinical needs through collaboration with your primary care provider and other members of your health team. This includes the management, monitoring and treatment of acute and chronic clinical needs as well as medication management. Residents are offered chef-prepared meals and enjoy laundry and housekeeping services.
Our Highest Level of Support
Enhanced Living Center
Directed by a Mayo Clinic physician, our Enhanced Living Center services are exclusively available to Charter House residents who require an increased level of nursing and personal care beyond those offered in our Catered Living Center. Our Enhanced Living Center offers private rooms with an inclusive package of services designed to adapt to your changing needs. The compassionate Charter House team will provide a secure, caring and engaging environment wherein residents can enjoy the fullness of life while having their evolving healthcare needs skillfully managed by trained professionals.
Our Enhanced Living Center package includes all Catered Living Center services and amenities in addition to our highest level of medical and personal support.
Charter House’s Enhanced Living Center package is designed to meet your current needs and adapt seamlessly to changing circumstances. Residents receive 24/7 personal care services delivered by one or more members of our highly-trained team, with routine access to a Mayo Clinic Physician and Nurse Practitioner. Residents living within Enhanced Living Center enjoy appointment coordination and scheduling, transfer and ambulation assistance, and dining assistance and monitoring.
Comprehensive Cognitive Impairment Training
All staff receive comprehensive cognitive impairment training to provide the critical knowledge, skills and compassionate approach required to meet the physical, emotional, and social needs of those living with memory loss.
PERSONAL, PROGRESSIVE CARE
5-Star Rated Short-Term Rehabilitation Center
Recover from hospitalization comfortably and reach your rehabilitation goals safely within our licensed Skilled Nursing Facility. With 25 private rooms, Charter House’s short-term rehabilitation care center offers comprehensive physical, occupational, and speech therapy services specifically tailored to each resident’s needs. Our skilled care team works diligently to plan personalized care, monitor progress, and adjust accordingly to help our residents recuperate with ease.
We partner with Mayo Clinic Hospice and Seasons Hospice to deliver hospice and respite when you need it most.

Dedicated Care Coordinators
You won’t embark on your rehabilitation journey alone at Charter House. Your designated Care Coordinator will be with you from admission to discharge, ensuring your transition in and out of the skilled care program is seamless and successful. By working closely with your physician, therapists and clinical staff, your Care Coordinator will plan your personalized care, communicate with you regarding your plan and progress, and help you understand your therapeutic services. Together with your full healthcare team, your Care Coordinator will guide you on the best path forward to your safe, successful transition home.
