Health care services at Charter House are designed to meet your needs as they arise. As a Continuing Care Retirement Community, Charter House offers access to the health care services you need — all under one roof. This range of services, called a continuum of care, is provided by a team of health care professionals that is available for consultation as your needs change. The goal is to allow you maximum independence in each of our venues within Charter House, while maintaining your safety, quality of life and good health.

Home Health Services

The goal of Home Health is to help you maintain good health and wellness, and remain in your retirement apartment as long as possible. Early engagement with our Home Health agency is key; it allows time for you to establish a relationship with a Home Health Registered Nurse Case Manager who will work with you and coordinate with your physician to understand your unique needs, goals, and medical history. The Registered Nurse Case Manager utilizes evidence based best practices to assure a high quality personalized plan of care. The Home Health program is based on a proactive wellness approach rather than a passive approach, and assures that experienced, knowledgeable nursing staff advocates and supports you to achieve your wellness goals.

Home Health services are provided under our Minnesota Comprehensive license and Medicare Certified Comprehensive license.

Assisted Living Center

Assisted Living Center at Charter House in Rochester MN

Apartments are available to Charter House residents, who after consultation with family, the physician and our health care team, agree to progress along the continuum of care. Assisted Living offers assistance with activities of daily living, as well as quality medical services, while maintaining the highest level of independence safely possible. One of the services offered to Residents is a Restorative Nursing Program.

The Assisted Living Center (ALC) provides an inclusive package designed to meet all your needs. The base daily fee includes:

  • On-site Registered Nurse 24/7
  • On-site Licensed Practical Nurse 24/7
  • 3 meals/day and snacks
  • Assistance as needed with ADL (Activities of Daily Living) such as dressing, grooming, etc.
  • Hourly rounding
  • Monthly assessment by nurse
  • Medication management
  • Centralized storage of medications
  • 24/7 staff response to in-room call light
  • Laundry
  • Housekeeping

Assisted Living services are provided under our Minnesota Department of Health Comprehensive license.

Supportive Living Center

Supportive Living residents enjoy private and semi-private suites and receive an increased level of nursing and personal care. Our Supportive Living Center provides an inclusive package designed to meet all your needs. The base daily fee includes all services listed for ALC. In addition, a Restorative aide is available 16 hours per day.

Supportive Living services are provided under our Minnesota Department of Health Comprehensive license.

The Short Term Rehabilitation Center

The Short Term Rehabilitation Center is comprised of 32 private and semi-private rooms certified by Medicare. Under the direction of a Mayo Clinic physician, the facility offers short term rehabilitation care and physical, occupational and speech therapy services following hospitalization. It also provides post-surgery rehabilitation services following major health events (these services are also provided in all service levels).

Continuity of staff ensures efficiency and high quality care that is accompanied by the concern and compassion of staff members who know you, your family and your physician.
