Health care services at Charter House are designed to meet your needs as they arise. As a Continuing Care Retirement Community, Charter House offers access to the health care services you need — all under one roof. This range of services, called a continuum of care, is provided by a team of health care professionals that is available for consultation as your needs change. In each of the care venues, the staff to resident ratio is higher than in most facilities. The goal is to allow you maximum independence in each of our venues within Charter House, while maintaining your safety, quality of life and good health.
Home Health Care Services
At Charter House we offer you the care you need to continue living a safe and satisfying quality of life in your apartment. Our home health care agency is licensed by the State of Minnesota and federally certified. Charter House offers an experienced team of Registered Nurses and certified Home Health Aides who help support your goal of optimum health.
Assisted Living Center
Rooms are available to Charter House residents, who after consultation with family, the physician and our health care team, agree to progress along the continuum of care. Assisted Living offers assistance with activities of daily living, as well as quality medical services, while maintaining the highest level of independence safely possible. One of the services offered to Residents is a Restorative Nursing Program.
The Assisted Living Center (ALC) provides an inclusive package designed to meet all your needs. The base daily fee includes:
- On-site Registered Nurse 24/7
- On-site Licensed Practical Nurse 24/7
- Home Health Aides 24/7
- 3 meals/day and snacks
- Assistance as needed with ADL (Activities of Daily Living) such as dressing, grooming, etc.
- Hourly rounding
- Monthly assessment by nurse
- Medication management
- Centralized storage of medications
- 24/7 staff response to in-room call light
The ALC is licensed by Minnesota Department of Health (MDH) as a Class F home care provider.
Supportive Care Center
Supportive Care residents enjoy private and semi-private rooms and receive an increased level of nursing and personal care. Our Supportive Care Center provides an inclusive package designed to meet all your needs. The base daily fee includes all services listed for ALC. In addition, a Restorative aide is available 16 hours per day.
Our Supportive Care Center is licensed by MDH as a Class F home care provider.
The Skilled Nursing Facility
The Skilled Nursing Facility is comprised of 32 private and semi-private rooms certified by Medicare. Under the direction of a Mayo Clinic physician, the facility offers skilled nursing care and physical, occupational and speech therapy services following hospitalization. It also provides post-surgery rehabilitation services and skilled care following major health events (these services are also provided in all service levels).
Continuity of staff ensures efficiency and high quality care that is accompanied by the concern and compassion of staff members who know you, your family and your physician.
